Birth Certificate

For people born in Massachusetts; the Birth Certificate is on file in the city or town in which you were born, the city or town in which your parents lived at the time of your birth, and the Department of Public Health Office of Vital Records. Click here for Request Form.

Death Certificate

Death Certificates are on file with our office for people who died in Southbridge and for those with Southbridge listed as their place of residence at the time of death. Click here for Request Form.

Marriage Certificate

Marriage Certificates are on file with our office for only those couples who applied for the license in Southbridge.

Ordering a Certificate

Certified copies of Vital Records (birth, death, marriage) are $10 each cash or check. Please call ahead to order all vital records.

Marriage License: You may apply for a marriage license at any City or Town Clerk's Office in the State. The license is only valid in Massachusetts. The record will always remain in the Town in which you filed. The couple MUST file their intentions together in person at the Clerk's office.

Birth, death, and marriage licenses are available using a credit card. Please visit the Southbridge Town Clerk’s Online Payment Center to pay.
